Get Ready For The 2019 CHEF Club Bus Trip To NYC!
Don’t miss out on the 2019 Bus Trip to NYC brought to you by the C.H.E.F. Committee. It’s on Sunday, December 8th. We leave Lancaster (Courtyard by Marriot in the Greenfield Business Park) at 8:30am promptly and get back around Midnight.
Cost is $65 per person. To get it split between 2 pay periods in November ($32.50 on 11/15 and $32.50 on 11/29), you’ll need to print out a Payroll Deduction form (Click HERE), complete it and have your general manager get it to Matt Pasciuta at CO by November 10th. Any form received after that will mean you’ll have 1 deduction of $65 on 11/29. All forms will need to be in by 11/24. Form can be faxed to Matt’s attention at 717-393-0955 or scanned and emailed to Matthew.Pasciuta@isaacsrestaurants.com. Then send the original to Central Office in Courier. If paying with check, make it payable to Isaac’s Deli, Inc. and send with form in Courier.
A Waiver Form for yourself and for each guest you are taking will need to be completed and sent in, too. Click HERE to print out form. Complete and fax to Johnny’s attention at 717-393-0955 or scan and email it to Johnny.Roberts@isaacsrestaurants.com Nobody will be allowed to board the bus without completing this form.
